IIR will provide sub-award management and oversight for the sub-award. Establish regular communication between and among IIR and NCPCV representatives to
ensure that the project stays on course and that any concerns, feedback, or upcoming
issues/risks are addressed in a timely manner and will support the implementation of the
deliverables.
• Create a subaward project dashboard or other electronic resources to post meeting
minutes, action items, and progress on deliverables that are readily accessible to all
parties.
• Have up to five business days to review documents, provide feedback, and issue
approvals for various aspects of the project. Should a longer time be required, IIR will
notify NCPCV. All external products for this project will be processed by the IIR Quality
Control Team.
• Establish protocols for use of the VALOR Program style guide and logos in the
NLEHWN project, as well as any required grant-related disclaimers.
• Conduct sub-award monitoring activities via emails, monthly status report and invoice
reviews, telecoms, and face-to-face meetings. IIR may decide to conduct random site
visits with NCPCV or the pilot sites.
NCPCV will do the following:
1.1 Costs Associated With the Subaward:
NCPCV is responsible for all costs towards the completion of deliverables, including any
related advisory boards costs (travel, per diem, and contract fees).

1.2 Invoices and Programmatic Reports:
Provide monthly invoices and written programmatic progress reports to IIR by the fifth
business day of the month. IIR will provide a template and documentation guidance for
NCPCV’s use. NCPCV will include monthly usage data in their monthly progress report. In
addition, NCPCV will highlight user feedback—positive and challenges as well as other
return on investment information—in the monthly progress reports.
1.3 Status Meetings:
Attend bi-weekly phone calls, or as determined by the program team, to provide status
information, discuss the subaward project, and provide access to the relevant parties on a
regular basis.
1.4 A Final Report:
At the conclusion of the project, provide a written overall project report to IIR.
1.5 Access to the Mobile App:
Provide access to the mobile health app to IIR for purposes of providing oversight and
management of the subaward. A nondisclosure agreement (NDA) will be signed by all
parties
