The Learning Collaborative Model is an approach designed to bring together practitioners on a set of common priority problems in order to learn promising and innovative strategies from expert faculty and one another. Through the model, expert faculty convene teams from different states and local municipalities for several facilitated calls, or "learning sessions," intended to accelerate the implementation, testing, and evaluation of promising practices, resulting in the rapid development of practical tools and strategies, between sessions.

During this period, the Urban Institute/Manatt Health convened three learning collaboratives for state and local municipalities from Maryland and California to develop practical tools and strategies states and localities can use to increase health coverage and connections to health care for justice involved people. The three learning sessions focused on, 1) linking people to coverage, 2) coordinated comprehensive systems of care, and 3) sustainable funding. The information shared is based on new and existing promising strategies gathered from states, local corrections and health care officials to develop and implement processes for connecting justice-involved individuals with health care.

The two sites consisted of a state agency and at least one county partner, were: 1) the state of California and Los Angeles County, and 2) the state of Maryland and Baltimore, Washington, and Harford counties. This TA also included numerous fact findings calls with practitioners across the country to assist the sites as well as develop materials for the learning collaborative sessions. During this period, Urban/Manatt helped stakeholders from both sites identify and reach consensus on the strategies they wanted to pursue.

Learning Collaborative #1: Linking to Coverage
Milestone Description

As part of the learning collaborative model, Manatt Health and the Urban Institute held the first learning session on strategies to link justice-involved individuals to health coverage. The teams presented an overview of 10 potential strategies to enroll justice-involved individuals in health coverage, highlighting implementation considerations for Medicaid and criminal justice agencies. State and county participants discussed the feasibility of each approach and reacted to the strategies. More than 40 representatives of 15 agencies from the State of California, Los Angeles County, State of Maryland, and Baltimore, Harford, and Washington counties attended the call.

Milestone Description

As part of the learning collaborative model, Manatt Health and the Urban Institute held the second learning session on strategies to develop coordinated, comprehensive systems of health care. The Manatt Health team presented an overview of 8 potential strategies, highlighting implementation considerations for state Medicaid and criminal justice agencies. Representatives from Maimonides Medical Center also presented an overview of the Brooklyn Health Home, which provides care management and coordination support for reentry populations. State and county participants discussed the feasibility and potential impact of the proposed strategies.
More than 30 representatives of 12 agencies from the State of California, Los Angeles County, State of Maryland, and Baltimore, Harford, and Washington counties attended the call.

The Urban Institute team presented an overview of potential strategies to finance Medicaid enrollment and systems of care, highlighting implementation considerations for Medicaid and criminal justice agencies. State and County participants discussed the feasibility and potential impacts of the proposed strategies. More than 35 representatives of 12 agencies from the State of California, Los Angeles County, State of Maryland, and Baltimore, Harford, and Washington counties attended the call.

Fact-Finding Calls with Practitioners Across the Country
Milestone Description

The Urban/Manatt team conducted a series of fact-finding calls to learn from practitioners across the country implementing innovative approaches to health care and Medicaid enrollment and financing mechanisms to inform the learning collaboratives and site-specific TA needs.
The team held a call with the Washington state Medicaid agency (10/23/2016) to learn about the agency's IT investments for suspending and reinstating Medicaid for the justice-involved population. The team also held a series of five calls with practitioners from Arizona about Medicaid enrollment activities and Medicaid/Corrections data exchange systems (conducted 10/28/2016; 11/8/2016; 11/17/2016; 11/21/2016; 11/23 with Pima County; Pima Community Access Program; Arizona Health Care Cost Containment System; Cenpatico, the state's Southern Regional Behavioral Health Authority; and Maricopa County).

Other calls included: a call with the Center for Medicare and Medicaid Services (11/14/2016) to discuss the feasibility of using 90% match to pay for Information Technology (IT) enhancements for Medicaid (to identify people with Medicaid coverage and people entering/leaving custody for the purpose of Medicaid suspension/reinstatement), deadline for cost allocation exceptions, and financing mechanisms to support enrollment and connection to care in the human services sector. The team held a call with the Connecticut Department of Corrections (11/21/2016) to discuss financing for eligibility specialists and processes for determining Medicaid eligibility. Finally, Urban/Manatt held a call with the Center for Prisoner Health and Human Rights, Rhode Island Department of Corrections, and the Rhode Island Department of Health and Human Services (11/16/2016) to learn about financing mechanisms for enrollment and identification of inmates who lack Medicaid coverage.

Discussion with Arizona AHCCCS
Milestone Description

The Urban and Manatt team organized a conversation for Maryland and Los Angeles County/California with representative from the Arizona Health Care Cost Containment System (AHCCCS). The discussion centered upon Arizona’s integrated IT system infrastructure, which facilitates connections between county jails and the state’s Medicaid eligibility and enrollment system.
